From March 2016 all patients in England, including children, will be allocated a named GP. At Laurel Bank you are welcome to see any GP, but if you would like to request a particular doctor to be your named GP, please complete the form below and we will amend your records.
Please note, you can still have an appointment with any other GP, or talk to a clinician other than your Named GP about your care.
