Subject Access Request under the Data Protection Act 1998
The practice is registered with the Information Commissioner’s Office and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act.
If you would like to view or have copies of your medical record, please complete our Subject Access Request under DPA Act 1998 form and return to the surgery either in person or by email to firstname.lastname@example.org. Your request will be processed within 21 days.
- In the interests of information security, your identity will need to be confirmed before your request can be processed. Please bring photographic identification (e.g. passport or driving licence) to the surgery, or email scanned copies with your request.
- If the GP is of the opinion that viewing information on your medical record may cause undue stress, the information may be withheld. Any decision to withhold information must be justifiable.
- If you believe a record to be inaccurate you may add a separate note to your record. Your original records, however, cannot be destroyed.
- The Practice has an agreed policy regarding data protection and security which is reviewed annually.
